On a cold start shake and loud exhaust noise, this is almost certainly an exhaust manifold leak, and it's one of the most common issues on older vehicles. When the engine is cold, metal contracts and any cracks or failed gaskets in the manifold open up, causing a loud ticking, rumbling, or rattling noise that's especially pronounced on startup. As the engine warms up and metal expands, the noise sometimes quiets down: Does yours settle after a few minutes of running? That's a classic telltale sign.
The shaking is likely related; a manifold leak disrupts exhaust backpressure and can cause rough running and vibration, especially at idle. Also check your exhaust manifold bolts, as broken or loose bolts are extremely common and cause identical symptoms. On many engines, these bolts corrode and snap off over time.
What year, make, and model is your vehicle? That'll help narrow it down further (some engines like Ford's 5.4L and certain GM V8s are notorious for manifold bolt failures specifically). Either way, get it inspected soon. An exhaust leak near the engine bay is a carbon monoxide hazard and shouldn't be left unaddressed. Hope this helps!
