On your 2003 Ford F-150 4.6L, running it on low fuel likely caused your current problems. And here’s why: Wwhen the tank gets very low, the fuel pump xxxx up sediment and debris that settles at the bottom of the tank, which can partially clog the fuel filter or damage the pump itself. This explains the stalling, the RPM surge before dying, and the intermittent running issue you’re experiencing now.
Start with the fuel filter - on the 2003 F-150 it’s located along the frame rail under the truck. If it’s never been replaced on a vehicle this old, it’s long overdue and is the cheapest first step. A clogged filter starves the engine of fuel under load and at idle causing exactly what you’re describing.
Next, test your fuel pressure - if the filter is clear but pressure is low or drops off under load, your fuel pump is likely weakened or failing from sucking up debris when the tank was dry. On the 4.6L you should see around 35-45 PSI at idle. Anything lower points directly at the pump.
Also check your IAC (Idle Air Control) valve - running rough on low fuel can foul this up and cause the erratic RPM surges and stalling at idle you experienced. It’s a cheap and easy clean or replacement on the 4.6L.
Start with the fuel filter - it’s the most likely and most affordable fix first.
Hope this helps.
