The image may show the lens foggy, but this is clear lens and a brand new part. The image picture reflection might have simply confused you. rest assured this is with a clear lens conforming to the oEM specifications and DOT/SAE certified part as well.

Are you referring to the header panel as a bracket? If so, no it is not included. This is just the headlight which is an OE (Original Equipment) direct fit replacement engineered to be exactly like the original. It will come as to how you see it in the product picture.
