The blower box is under hood left top side , black plastic housing with fan on front side and resistor on top, held on with two screws. It plugs into existing harness. My 2002 3500 express has 4 switched positions , works as supposed to.
Had to enlarge mounting hole , cut plastic with something seal mount onto box,not a problem.

On your image, the green card occupies less than half of the length of the black plastic holder. On my old resistor, the green card runs the full length of the black holder.
