Ordered for my 2001 4Runner with california emissions (2 cats). The flange has a pipe in the middle that exceeds and inserts into an opening on the headers. I believe that this piping sticks out too long, and caused a pretty gnarly gap between the flanges (OEM cats has a flush seal). The predrilled holes on the new Cat flange did not fit the header studs and caused some issues. Had to extend/widen all holes for it to even allow the studs to insert. 1 Week later and it is still holding up well. Pretty happy with the outcome, and some modifications are expected as it is a 25+ year old car.

The upstream O2 sensor is not located on this part of the catalytic converter. it's higher up on the part that this set attaches too closer to the engine. the o2 sensor on this part is the downstream o2 sensor.

My 2011 Equinox uses two 02 sensors as well. One of the O2 sensors, (downstream 02) screws directly into the hole directly under the Catalytic Converter unit, whereas the other 02 sensor screws directly into the took of the header or exhaust manifold itself.

No you probably have an exhaust leak. Recommend taking it to a muffler shop and have them inspect

Yes, but cat converter would be more of a rotten egg/sulfur smell. You may have an exhaust leak. Is the car loud?
