This heater core says on the notes it does not come with the o-rings. This is a guaranteed fit for your2004 Dodge Caravan, all submodels, 4 or 6 Cyl 3.3L as per checking.

Yes! When you install the new heater core the O rings are needed. Inspect the O rings on the heater core that you removed. I was able to reuse the O rings. But it would be a good idea to replace them just to get a good seal.
