We suggest getting those washers locally through a local auto supply store. They sell them at most auto parts stores.

Usually I find two tabs on the sides of the trim piece and then get them started into the hole ion the door panel.. It sometimes then takes quite a bit of force to get them to lock into the metal inner skin of the door, depending on the thickness of any aftermarket trim panels that were put on possibly as part of a conversion. Mine has pretty thick inner trim panels and I find it takes a bit of pounding with a fist to get the tabs past the panel and locked into the hole in the metal door skin.
